In-Home Assessment and Evaluation

Following a phone consultation, we will come to the home of the individual needing care to do a comprehensive assessment and evaluation, which includes a review of:

  • Physical and mental limitations
  • Strengths
  • Social interests
  • Outside contacts
  • Sensory loss
  • Medical history and status
  • Nutritional status
  • Safety of environment
  • Legal and financial planning
  • Health insurance quality
  • Cost control

We provide ongoing service only after we have evaluated your needs and have explained a plan of service with the expected results and cost of services.

Care Planning

Developing a Care Plan to follow is crucial to address immediate and long term needs. Becoming the caregiver of an aging loved one is new territory for almost everyone. There are many things to learn, with the challenge of knowing where to begin if you even have the time to begin!  Long distance caregiving presents even greater challenges.  There are things you need to prepare for ahead of time, but without the experience, how can anyone expect you to know what to do? Care Coordinating and Management

Most of us fear the loss of independence as we age and dread the idea of having to depend on others for our needs.  Elder Care Management Solutions helps elders and their families to have more control of their care, and we treat them in a special way…with privacy, flexibility, quality and a personal touch. Client wishes are balanced with their needs; we carefully customize a plan to assure that all needs are met, with an eye on safety.  The services that we can coordinate or provide include:

Crisis Intervention

Need more peace of mind? We are available 24 hours a day, 7 days a week for the urgent needs of our clients and their families. We can advocate in an Emergency Room, provide the Health Care Proxy quickly, and have approaches ready in the event of power outage or flood evacuation.

Choosing Nursing Home Care

There may come a time when your loved one’s needs become greater than what can be safely met at home or financially maintained at home. This is where our firsthand knowledge of local nursing homes, adult care facilities, assisted living facilities and other independent living options will be valuable to you. We streamline the research process for you and present the options that best meet your needs and preferences. We advocate and assist with the application process and other requirements before, during and after admission. We can coordinate all of the following:

  • The move to or from the home, retirement complex, or nursing home
  • A move in or out of state
  • Assistance with applications
  • Transferring medical records
  • Arranging for packing and moving belongings
  • Having the room ready upon arrival

Money Management

Our services can include assisting older persons in the management of their personal finances. We will come to them in their own homes to balance their accounts, pay their monthly bills and maintain their financial records. This is just one more thing that we can do to ease your mind and save you time. We can also offer step by step guidance in Medicaid application completion as well as represent the applicant in the application interviews.
